This referral typically has your information along with indications of your injuries (diagnostics). Your doctor can give this referral to you, or they can fax or e-mail it to us.
The first time you see us, we will have some paperwork for you to fill out. Be sure to bring your insurance claim letter and auto insurance card with you so we can make copies of both.
